How do I know whether my medical or dental care claim has been settled?
You may find out the status of your claims by viewing the “My claims” section in your personal online account. Only medical and dental claim forms will appear online. Please contact us should you require information about any other type of claim.
How do I obtain the list of my incurred medical and dental expenses for income tax purposes?
The complete list of your medical expenses for non-refundable tax credit purposes is available in your personal online account under “My claims” only once the calendar year has elapsed. In this section you have access to the list of your medical expenses for all previous years since your enrollment in the plan.
Is my personal information secure?
Yes. Your personal information is stored on our secure server so other web users do not have access to your file.
Why can’t I find any information on my life or disability insurance online?
In order to ensure the privacy of your personal information, this information is only available if you contact us directly.
What benefits am I entitled to?
Information booklets are distributed to members. We suggest you consult it upon reception as it describes all the benefits you have under your group plan and include an easy-to-use table of contents that will help you find the section you are looking for.
Each insurance policy has different exclusions or limitations. Please be sure to read carefully and understand them.
How do I submit a medical or dental expense claim?
All claims must be made in writing on standard medical or dental claim forms. Additional information and claims forms are available under the “Forms” section of this Website.
Fill out the claim form, sign it and attach all original receipts to the completed claim form before sending it to us by mail. The claim office address is located on the claim forms.
Please remember to take a copy of all receipts and forms before you submit as they will not be returned to you.
Medical or dental claims are normally processed within five working days following reception. Complex cases may require more time.
How do I change my personal information?
You may consult your personal information under “My File”. You may also make some modifications under that section by clicking on the desired change. A request will be automatically send to GPM with your up-to-date information.
Please contact your group plan administrator should you wish to make any modification to personal information that is not available through the Website.
What is meant by a “deductible”?
The deductible is the portion of eligible expenses that must be incurred and paid by you or your dependants before any benefits are payable.The maximum deductible required is specified in your booklet of Benefits. Deductibles are applied on a calendar-year basis, i.e., from January to December.
